The patient is here for initial evaluation for EMG for tingling and numbness in the right arm and weakness in the right arm. As you know, the patient is as pleasant male who slipped and fell on Wal-Mart Supermarket on September 20, 2011. After that the patient was taken to Raritan Bay Hospital and was diagnosed to have a fractured clavicle and the patient is complaining of tingling and numbness in the right arm. He denies any pain in the neck. He also has pain in the knee and ankle. He is being followed by Dr. . The patient describes the pain as 5/10. He describes the pain as tingling and numb on right leg. He has not started any physical therapy for fractured clavicle. He is still using the strap, taking naproxen and Aleve on as needed basis to help in pain. He was prescribed Vicodin by Dr. and was prescribed Ambien for sleep.
Tingling and numbness in the right arm, status post fractured clavicle.
Rule out traumatic neuropathy or brachial plexopathy.
I will schedule the patient for NCV/EMG studies of the upper extremities as requested by Dr. . The patient is advised to follow up with Dr. as advised.
