Lumbar spondylosis

The patient is here for follow-up visit for chronic low back pain and numbness and tingling of both lower extremities. The patient also had a history of L5-S1 laminectomy in January 2000 with improvement of some pain. The patient states she still has significant amount of pain. Pain level is 6/10. She has started Vicodin that seems to be helping her lot. She tries to avoid taking it as much as possible but she needs it to help in pain. She still has few days of medication left. She denies any memory impairment, sedation, nausea, itching, or high with the medication. She denies any bowel or bladder dysfunction. She denies any changes in her past medical history, surgical history, medication history, or drug allergy.

Lumbar postlaminectomy syndrome.
Lumbar spondylosis.
Lumbar spinal stenosis.
Lumbar radiculopathy.

The patient will continue with Vicodin 5/500 one tablet every six hours as needed for pain, #100 tablets were dispensed to be filled on 12/09/2011. We will see the patient back in follow-up in a month.
