Failed back surgery syndrome

The patient is here for followup visit for his neck and lower back pain. His pain level is again 6-7/10 on a visual analog scale. With pain medications, the pain is well controlled. He is going to be scheduled for a transforaminal injection two weeks. With the help of the medication, he is alert and functional with better quality of life. Otherwise, there are no other changes in medical history or social status, or medications since the last visit.

DIAGNOSES:
Failed back surgery syndrome.
Lumbar radiculopathy.
Cervical radiculopathy.

RECOMMENDATIONS:
The patient is to continue with oxycodone 30 mg one tablet q.6h as needed for pain. We will see the patient back in followup in 30 days. The patient will continue with current medications.
